Medicare and Insurance

Do you accept Medicare for home health services?
Yes. Health at Home is a Medicare-certified Home Health agency serving Southeast Florida. If your physician orders home health care and you meet Medicare’s eligibility criteria (homebound status and a need for skilled care), Medicare covers 100% of your home health benefit with no out-of-pocket cost. This includes skilled nursing visits, physical therapy, occupational therapy, speech therapy, and medical social work provided in your home. Our team handles all Medicare authorization and coordination on your behalf.

Yes. We work with most long-term care insurance policies for private duty caregiving and private duty nursing. Our team can help you verify your benefits and navigate the claims process before care begins, at no charge. For Medicare home health, there is no cost to eligible patients. For private pay families, non-medical caregiver rates in Southeast Florida typically range from $35 to $40 per hour depending on hours, schedule, and level of care. 24-hour care is priced on a daily or weekly rate. We offer flexible scheduling from a few hours per week to full-time care so families can match coverage to their budget.

Our Services

What is the difference between home health care and private duty care?
Health at Home provides both services. Medicare-certified Home Health is physician-ordered skilled care. Our nurses and therapists visit your home to treat a specific medical condition or support recovery after hospitalization. It is fully covered by Medicare when you qualify. Private Duty care comes in two forms: Private Duty Nursing, provided by RNs and LPNs for patients requiring extended skilled nursing beyond what Medicare covers; and non-medical caregiving, where our trained caregivers assist with personal care, companionship, meal preparation, housekeeping, medication reminders, and daily activities. Many families use a combination of all three services as needs evolve.
Yes. Our Private Duty non-medical caregivers provide compassionate in-home support for seniors who need help with daily activities but do not require skilled medical care. Services include personal care (bathing, dressing, grooming), companionship, meal preparation, light housekeeping, medication reminders, transportation, and errands. Care can be arranged for a few hours per week up to 24-hour care. All caregivers are background-checked, bonded, insured, and direct employees of Health at Home.
Yes. Health at Home provides Private Duty Nursing throughout Southeast Florida. RNs and LPNs are available for extended hourly or 24-hour shifts. This is ideal for patients requiring ongoing clinical supervision beyond what Medicare’s episodic visits provide: wound care, IV therapy, ventilator management, post-surgical monitoring, catheter care, and chronic disease management. Billed through long-term care insurance or private pay.
Through our Medicare home health program: wound care, medication management, IV therapy, disease management education, post-surgical care, and physician coordination. Through Private Duty Nursing: RN and LPN coverage for extended hours for patients requiring ongoing clinical supervision. Skilled therapy services, including physical therapy, occupational therapy, speech therapy, and medical social work, are also available in the home.

Yes — we specialize in dementia and Alzheimer’s care throughout Southeast Florida. Our caregivers receive specific training in memory care: maintaining safe routines, managing behavioral symptoms, reducing anxiety, and keeping patients safely at home. For advanced cases, our Private Duty Nurses can provide clinical oversight alongside the caregiving team. We work closely with the patient’s family and physician to build a care plan that evolves with the condition.

Getting Started & Service Areas

How quickly can home care begin after I contact you?

We work to begin care as quickly as possible, often within 24 to 48 hours of your initial call. For Medicare home health, we coordinate directly with your physician to obtain orders and complete the admission process efficiently. For private duty caregiving or nursing, care can often begin the same day or next day. Both offices are available 24 hours a day: Boynton Beach (561) 806-7027 · Port St. Lucie (772) 288-7386. Or contact us online and we will reach out promptly.

Absolutely. Every caregiver, aide, and nurse at Health at Home undergoes criminal background checks, reference verification, skills assessment, and in-person interviews. All clinical staff hold active Florida licensure. Our staff are bonded and insured, and we carry full agency liability coverage. All staff are direct W-2 employees. We do not place independent contractors. Every person who enters your home is fully covered under our agency insurance and supervised by our clinical team.

We serve Southeast Florida from two offices:

Boynton Beach serves Palm Beach County and Broward County, including Boynton Beach, Boca Raton, Delray Beach, West Palm Beach, Wellington, Lake Worth, Pompano Beach, and surrounding communities. Call (561) 806-7027.

Port St. Lucie serves St. Lucie County, Martin County, and Indian River County, including Port St. Lucie, Stuart, Jensen Beach, Palm City, Hobe Sound, Fort Pierce, Vero Beach, and Sebastian. Call (772) 288-7386.

Both offices are available 24 hours a day.

Yes. Our Port St. Lucie office serves all of Indian River County, including Vero Beach and Sebastian, with Medicare-certified home health, Private Duty Nursing, and non-medical caregiver services. Call (772) 288-7386 to speak with a care coordinator 24 hours a day.

Health at Home has served Southeast Florida seniors for over a decade with a 5-star Google rating across both locations. We are one of the only agencies in Southeast Florida offering all three levels of care under one roof: Medicare-certified home health, Private Duty Nursing, and non-medical caregiver services. Your loved one’s care can scale seamlessly as needs change, without switching agencies. We are locally owned and operated, clinically led, and serve Palm Beach, Broward, Martin, St. Lucie, and Indian River counties.

Transitions & Scheduling

Can you help with home care after a hospital discharge or rehab stay?
Yes. Hospital-to-home transitions are one of the most common reasons families contact Health at Home. If your physician or hospital discharge planner orders home health services, we can begin Medicare-certified skilled nursing and therapy visits within 24 to 48 hours of discharge. For patients needing ongoing support beyond what Medicare covers, such as personal care, meals, or companionship, our private duty caregivers can be in place the same day. We work directly with hospital case managers and discharge planners across Palm Beach, Broward, Martin, St. Lucie, and Indian River counties.
When a Medicare home health episode ends, many patients still need ongoing support, just not at the skilled care level Medicare requires. Health at Home makes this transition seamless because we provide all levels of care under one roof. Once your Medicare benefit concludes, we can transition you directly into private duty non-medical caregiving or Private Duty Nursing if clinical oversight is still needed, with no new agency intake and no gap in care. Your care coordinator will discuss your options before the Medicare episode closes.

Health at Home does not provide Live-In caregivers.

Yes. Health at Home provides care seven days a week including evenings, overnight shifts, and weekends. Our office is staffed 24 hours a day. For patients needing overnight supervision, we offer both awake overnight caregivers and sleep-over arrangements depending on the level of oversight required. Weekend and holiday coverage is available across all of our service areas in Palm Beach, Broward, Martin, St. Lucie, and Indian River counties.
Yes. Respite care is one of our most requested services. If you are a family member or spouse who is the primary caregiver, Health at Home can provide a trained caregiver for a few hours, a full day, overnight, or an extended period so you can rest, travel, or attend to other responsibilities. Respite care can be scheduled on a recurring basis or as needed, often beginning within 24 hours of your call. Available throughout all of our Southeast Florida service counties.

Specific Situations & Populations

Do you provide companion care services for seniors?
Yes. Companion care is a core part of our non-medical private duty caregiving services. Isolation and loneliness are serious health risks for seniors, and our companion caregivers provide meaningful social interaction, conversation, accompaniment to appointments, help with hobbies, and a consistent friendly presence at home. Learn more about our companion care services or call to discuss scheduling. Available from a few hours per week to daily visits across all of our Southeast Florida service counties.

While the majority of our patients are seniors, Health at Home serves adults of all ages who need home health or private duty care. Our Medicare-certified home health services are available to any Medicare-eligible patient regardless of age. Our private duty nursing and non-medical caregiving services are available to adults of any age dealing with illness, injury, disability, surgery recovery, or chronic medical conditions. Call us to discuss your specific situation.

These are frequently confused. Medicare-certified home health is designed to treat, rehabilitate, or manage a medical condition with the goal of improvement or recovery. Hospice, by contrast, is for patients with a terminal diagnosis and a life expectancy of six months or less, focusing on comfort rather than curative treatment. Health at Home provides home health and private duty services. We do not provide hospice. However, if your loved one is on hospice, our private duty caregivers can complement hospice services by providing personal care and companionship that hospice agencies typically do not cover.
