For more than 25 years, our team at Health at Home has helped South Florida families answer a question that rarely has a simple answer: What does the right care look like for someone who wants to stay at home?
Sometimes that question comes up gradually. Maybe a parent is still independent but has stopped driving, meals are getting harder to manage, or a spouse is taking on more caregiving than they reasonably can alone.
Other times, everything changes in a day. There’s a fall, an unexpected hospitalization, a surgery, a new diagnosis, or a discharge plan that suddenly leaves a family asking, “What do we do now?”
Those are the moments our company was built for.
At Health at Home, we provide Florida home care across Palm Beach, Broward, Martin, St. Lucie, and Indian River counties. But what makes our model different isn’t simply the number of services we offer.
It’s how we put those services together, how seriously we take continuity of care, and how personally we approach the people trusting us to come into their homes.
We’re a Florida Home Care Agency, Not a Caregiver Registry
One of the most important distinctions families should understand when comparing Florida home care providers is the difference between an agency and a registry.
Health at Home is an agency.
Our caregivers are our employees. They’re screened, background checked, insured, trained, supervised, and supported by our organization. Families aren’t left to independently manage a caregiver relationship or figure out what happens when something changes.
There’s a company and a care team behind the person walking through your door.
That matters when a caregiver has to call out. It matters when a client’s condition changes. And it matters when care becomes more complex than one individual caregiver can reasonably manage.
We believe home care should come with accountability. If something isn’t working, we want to know. If needs change, we revisit the care plan. If a caregiver notices something unusual, there’s a team they can communicate with.
Our clients aren’t hiring one person and hoping everything works out. They’re working with an organization responsible for the bigger picture.
Florida Home Care and Home Health Care, All in One Place
Families don’t always know whether they need “Florida home care” or “home health care,” and frankly, they shouldn’t have to become experts in the terminology before they can ask for help. At Health at Home, we provide both, so our team can help families understand what kind of support makes sense now and what options are available if needs change later.
Our private-duty home care services support everyday life and can include:
- Personal care
- Companion care
- Dementia and Alzheimer’s care
- Household assistance
- Meal preparation
- Transportation and errands
- Help with daily routines and activities
- 24-hour home care
For qualifying patients who need clinical care at home, our Medicare-certified home health services can include:
- Skilled nursing
- Physical therapy
- Occupational therapy
- Speech therapy
- Medical social work
Having both available through our company becomes especially valuable when someone’s needs change. A client might begin with companion care a few days a week and later need more personal support. Someone else may come home from the hospital needing skilled nursing and physical therapy, then continue with private-duty assistance after the Medicare-covered episode ends.
Instead of sending families back to square one every time the level of care changes, our model gives them access to different types of support within the same organization. We’re also ACHC accredited and Medicare-certified, reflecting the clinical and operational standards behind our home health services.
Continuity of Care Is Something We Pay Attention To
Continuity sounds clinical, but in practice it’s very personal.
It means not having to explain your loved one’s situation from the beginning every time something changes. It means having people involved in care who understand the history, the routines, the family dynamics, and what “normal” looks like for that individual.
It also means recognizing that consistency doesn’t always mean one caregiver providing every type of support indefinitely.
Different stages of care may require different people.
A companion caregiver, a home health nurse, a physical therapist, and a caregiver providing extensive personal assistance all bring different skills to the home. Our job is to help make those transitions feel connected rather than fragmented.
Whenever possible, we prioritize caregiver consistency because familiarity is incredibly valuable. A caregiver who spends regular time with someone learns how they like their coffee, what time they usually get dressed, how talkative they normally are, whether they tend to nap in the afternoon, and a hundred other details that would never appear on a care plan.
That familiarity also helps someone notice when something feels off.
At the same time, we have a larger team available when a client’s needs require a different level of support. The goal isn’t consistency at the expense of appropriate care. It’s thoughtful continuity, with the right people involved at the right time.
We Take Caregiver Placement Personally
Home care is incredibly personal work.
We’re asking someone to welcome another person into their home, often during a vulnerable season of life. That caregiver may help with bathing, dressing, meals, transportation, mobility, or simply provide company during hours that would otherwise be spent alone.
So we don’t think caregiver placement should be treated like filling an open shift.
Availability matters, of course. But personality, communication style, and the type of support being provided matter too.
Some clients love conversation. Others enjoy having someone nearby without feeling like they need to entertain them. One person may want encouragement to get out of the house, while another feels happiest maintaining a quiet routine at home.
We want to understand those things.
No placement process can guarantee that every personality will click perfectly on the first try, and we’re candid about that. What we can promise is that we pay attention, communicate, and make adjustments when needed.
Our Approach Is Pretty Simple: People Helping People
Home care can become complicated quickly. There are insurance questions, Medicare requirements, care levels, schedules, family dynamics, diagnoses, discharge instructions, and a lot of unfamiliar terminology.
We know families often come to us while they’re overwhelmed. Our job isn’t to make the process feel more complicated.
We try to be real with people. We’ll tell you what we can do, what we can’t do, what type of care may make sense, and when another resource might be a better fit.
There’s no benefit in overselling care someone doesn’t need or pretending there’s a perfect solution when there isn’t one.
We’re a company made up of people helping other people through situations that can be stressful, emotional, and sometimes messy. We take the responsibility seriously, but we don’t believe that requires corporate language or a sales pitch.
Sometimes a family just needs someone knowledgeable to say, “Here’s what I’m seeing, and here are your options.” That’s the kind of relationship we want to have with the people who call us.
We Start With a Free In-Home Assessment
One reason we offer a free in-home assessment is that care can look very different once you’re actually standing in someone’s living environment.
A phone conversation may tell us that someone has been “a little unsteady lately.”
Seeing that person move through the home can tell us much more. Are there stairs? Is the bedroom far from the bathroom? Are they holding onto furniture while they walk? Is food being prepared regularly? Are medications organized? Does a spouse appear exhausted from trying to do everything alone?
Our Florida home care assessment gives our team the opportunity to meet the person, understand what they can still do independently, see where support may be useful, hear the family’s concerns, and talk through the options together.
We don’t want to recommend care based solely on a checklist over the phone when we have the opportunity to understand how someone is actually living.
The assessment also gives families the chance to ask questions before making decisions.
Sometimes they leave knowing they need care immediately. Sometimes they realize they need more support than they expected. Sometimes the conversation helps them prepare for what may be coming next. All of those outcomes are useful.
When Florida Home Care Is Needed Quickly, We Move Quickly
Not every family has weeks to plan. We regularly hear from people after an unexpected hospitalization or medical event who suddenly learn that a parent or spouse is coming home and shouldn’t be alone.
In situations like these, the timeline changes fast.
Our team is structured to move quickly, and depending on the circumstances, can begin services within 24 hours.
That responsiveness is important to us because we understand what’s happening on the other side of the phone call.
Someone may be sitting in a hospital room trying to coordinate discharge. An adult child may be booking a last-minute flight to Florida. A spouse may have just realized that helping someone transfer or move safely around the house is more than they can manage alone.
When care needs to happen quickly, our focus is on figuring out what’s necessary, what resources are available, and how soon we can safely put the right support in place.
Our South Florida Relationships Are Part of the Care
We’ve served South Florida families for more than 25 years, and our work extends beyond private residences.
Health at Home serves clients throughout Palm Beach, Broward, Martin, St. Lucie, and Indian River counties, and we’ve developed relationships with assisted living facilities, medical professionals, and other organizations involved in caring for older adults throughout the region.
We can provide Florida home care in private residences as well as assisted living settings when additional one-on-one assistance is appropriate.
We also work alongside medical providers and other professionals involved in a client’s care. Our role may be different from theirs, but everyone benefits when communication is clearer and families don’t feel like they’re coordinating an entire care system on their own.
Let’s Figure Out What the Right Florida Home Care Looks Like for You
You don’t need to know exactly what type of Florida home care you need before you call us. In fact, many families don’t. You may simply know that something has changed, home is becoming harder to manage, or you’d feel better having another set of eyes and hands involved.
That’s where we can start. Our team can talk through what’s happening, answer your questions honestly, and schedule a free in-home assessment so we can understand the situation in the environment where care will actually happen.
Whether you’re planning ahead or trying to put support in place quickly after an unexpected change, reach out to our team. We’ll help you understand your options and determine what makes sense for you and your family.
